Residential Services
Personal Assistance Services/Habilitation - Clients receive support services allowing them to live as independently as possible with their family or in their own home. Support staff provide assistance with housekeeping, meal preparation, medication administration, personal care, transportation, and other independent living activities. The time spent providing services each week is based on the needs of the client and their family.
Host Home Companion Care* - [Formerly known as Foster Care] Provide direct personal assistance with activities of daily living, assistance with housekeeping, meal planning and preparation, securing and providing transportation, reinforcement of counseling and therapy services, and assistance with medications. FCC providers also facilitate inclusion in community activities, social interaction, participation in leisure activities and development of socially valued behaviors. Supervised Living* - Provided in a residence usually housing 2-3 individuals. Supervised living is provided by support staff who care for the residents as needed during hours when they are awake.
Residential Support Services* - Provided in a residence, usually housing 2-3 individuals. Support staff provide services as needed around the clock.
Minor Home Modifications - Provided to individuals requiring reasonable structural modifications within their home to improve their overall living environment by making it more accessible (bathroom rails, access ramps, widened doors, etc.).
